**Once you submit this form, you will immediately receive a 'confirmation' email that we received your form. If you were in the system last year, when you receive the email link from Verkada, you may not have to download the Verkada Pass App, just logging in should work. If you are new to our Access system, you will receive an email soon from Verkada for the email link.
1) Type first and last name of the enrolled child in Before Care/After Care; one child per box. 2) If you only have 1 child, leave the remaining boxes empty and click 'Submit'.
